How to Make a Restaurant Survey with Google Forms and a QR Code

Google Forms can be used to create a restaurant survey and share it through a QR code. Check the form settings and try the respondent view before displaying it in the restaurant.

Create the form and add focused questions

Start a new form, add a title that explains its purpose, and choose question types that guests can answer quickly.

Review access and response settings

Check the form from a guest’s point of view. Make sure the sharing settings allow the intended respondents to open it, and confirm whether any account sign-in or response limit is enabled.

Share the form and make a QR code

Use the respondent-facing link rather than an editing link. Turn that address into a QR code and test the code on a phone that is not signed in to the form owner’s account.

Add a clear invitation in the restaurant

Place the QR code where it can be scanned and explain what it opens. Avoid collecting email addresses or other personal information unless there is a clear need.

Decide who will check responses

Choose who will open the response view, when they will check it, and how the restaurant will use what guests share.

Check before putting the form out

Open the form as a respondent, submit a test response, and check the QR code and access settings from a phone.

1. Create a form with a clear title

Open Google Forms and choose a blank form. Give it a name guests will recognize, such as ‘[Restaurant name] Guest Survey,’ and explain its purpose briefly.

2. Start with about three questions

A first version can ask for an overall impression, the part of the visit that stood out, and an optional comment. Google Forms supports choice, checkbox, and written-response question types; choose the format that fits what the restaurant will review.

3. Check whether guests must sign in

For a restaurant survey, verify the account requirements carefully. Google’s ‘Limit to 1 response’ setting requires respondents to sign in to a Google Account. Do not turn it on unless that is intended and clearly explained.

4. Publish the form with the intended access

When the questions are ready, make the form available to respondents and review the access scope. Product settings can change, so inspect the current respondent view and sharing options before printing the QR code.

6. Turn the respondent URL into a QR code

Create the QR image from the guest-facing URL. Put a short explanation beside it, such as ‘Tell us about your meal today,’ so guests know what the code opens.

7. Complete a phone test before printing

Scan the code yourself and go through the form from beginning to submission. Check text, buttons, required questions, sign-in prompts, and the confirmation after sending.

8. Place the QR code where it fits

After the test succeeds, print the sign and choose a location such as a table, the register, or a card included with takeout. Make sure guests can scan it and that it does not interrupt service.

9. Decide how to review answers

Google Forms has a Responses view for summaries and individual submissions. You can connect a spreadsheet or work with a CSV when that fits the process. Saving answers is not the same as using them: name who checks the responses and when.
